Group Request and Other Helpful Forms:

	Group Information Sheet - please use this form to provide us information about your group and to specify your needs including lodging, food service, and more.
	Environmental Stewardship Retreat Form - if you are interested in having an environmental stewardship retreat at Camp Gray, please use this form.
	Spiritual Retreat Form - if you are interested in a spiritual retreat facilitated by Camp Gray staff, please use this form.
	Chapel Use and Inventory - we have a dedicated chapel on Camp Gray property. If you are interested in using the chapel for worship, this form provides a list of all the items that Camp Gray provides in the chapel as well as a list of what you would need to bring for your group.
	St Vincent Retreat Center Layout - this floor plan of the retreat center provides the locations and capacity for each of the rooms in Vinnies.

Group Rental Forms:

	Camp Gray Group Introduction and Guidelines - this form contains an introduction to camp and the basic guidelines we ask you to follow during your stay.
	Facility Usage/Indemnity Agreement - to be filled out and signed by the leader of the facility user.
	Individual Waiver/Indemnity/Hold Harmless Agreement - form to be filled out by the individual members of a user group.
	Liability Insurance Information - this form contains information regarding the insurance user-groups must carry in order to stay at camp as well as information about the special events coverage offered by Camp Gray.
	Application for Special Events Coverage - please use this form if you would like to apply for special event insurance coverage from the Diocese of Madison for your group (for an additional fee)
	Departure & Cleaning Checklist - please fill out this form at the end of your stay to provide us with accurate information about your group and to read our cleaning request.
	Play it Safe Sheet - this sheet provides important safety information to keep in mind while enjoying your stay at Camp Gray.
	User Group Pool Safety Regulations - this form contains safety regulations specific to the use of the Camp Gray pool.
	Online Rental Group Evaluation - (electronic form) Once you have completed your stay at camp, please fill out this form to let us know how we can serve you better. We appreciate your feedback.
	Rental Group Evaluation - (paper form) Once you have completed your stay at camp, please fill out this form to let us know how we can serve you better. We appreciate your feedback. (Alternatively, you can fill out the Online Rentals Survey)
